The cost is for the test only.  This test requires a blood draw which can be done in clinic or arranged locally by you and your local provider.     To go over this test with Dr. Lelwica, you will need to book a laboratory review under New Patient if you have not seen her before, or existing patient if you have already had your  new patient intake first visit.   Shipping is $10 and will be invoiced to you separately.  Shipping only available in the US or APO.
